One question I have - The anti-tip crowd always compares to Europe and the rest of the world. Especially, in places where it is cheaper than the US, what gives from EU to US where prices are lower AND you aren't expected to tip 20%. Yes salaries are lower, but I'm sure servers make a living wage. I'm sure you can also point to the reduced service-levels you mentioned.

I think my line for tipping is usually - a) if I'm standing up when I order and/or b) if I've tried the food prior to tipping.

Great article. Also worth mentioning the automatically added separate 15-20% service charge that sometime happens even before the tip is requested.

Restaurants in Miami Beach were reprimanded for not announcing those extra charges 10 years ago but they’ve snuck their way back onto bills.

For those swivel pay things, keep in mind you can always add a custom tip amount. I find that the best way to tip an amount I feel is appropriate given the level of service.
